AFJROTC Information For Parents

The following information was sent home with cadets on the first day of school.  If you need another copy please click on the link and then print, sign, and return.  These forms are all due 9 September 2011!

  1.  Parents Memorandum of Understanding:  This form is the statement that it is the cadet’s responsibility to maintain all uniform items in a clean and orderly condition during the school year and when the uniform is turned in. Per AFJROTC requirements, cadets are obligated to wear the uniform properly and proudly. In doing so, cadets uphold the dignity of the Air Force, their school, fellow cadets, and themselves.

  2.Cadet Health/Wellness Program:  AFJROTC Wellness Program is designed to work with your child to help them improve their physical fitness. All physical activity sessions will be supervised and monitored by at least one of our instructors. These sessions include walking, running, calisthenic exercises, stretching, team sports and the low and high elements challenge courses. The AFJROTC instructors have been trained in administering CPR if needed.  

  3.  Web Photo Permission Slip: To increase Cadet involvement with our website, we’d like to publish pictures of the Cadets doing corps-related activities on the site. We need your permission to do this.

4.  Transportation Permission Slip: There are many AFJROTC school and community events that may require transportation to and from an event. The transportation may require the AFJROTC instructors and cadet parents to assist with transportation to and from these events.

5.  Volunteer Background Check Form:  If you are interested in volunteering to attend any of our trips, events, or activities as a chaperone you MUST have completed one of these forms and turned it in to Lt Col Davis or MSgt Olson.
